Aetna Group Life Insurance Program for Jackson County Schools
BASIC EMPLOYEE LIFE INSURANCE -
Jackson County Schools provides all Full-Time Active Employees with $15,000 of Life insurance and $15,000 of Accidental Death and Dismemberment insurance
*Active Work Rule: If you happen to be ill or injured and away from work on the date your coverage would take effect, the coverage will not take effect until you return to full-time work for one full day. This rule also applies to an increase in your coverage.
OPTIONAL EMPLOYEE LIFE INSURANCE
This insurance is paid for by the employee and is available in the amounts listed below. *Active Work Rule: If you happen to be ill or injured and away from work on the date your coverage would take effect, the coverage will not take effect until you return to full-time work for one full day. This rule also applies to an increase in your coverage.
OPTIONAL DEPENDENT LIFE INSURANCE
Spouse and child (ren) coverage may be purchased only if the employee is covered. The amount of Spouse or Child coverage must not exceed 50% of the employee’s optional life coverage amount.
You may cover your:
- wife or husband; and
- unmarried children who are over 14 days but under 19 years of age. Any other unmarried child under age 26 who goes to school on a regular basis and depends solely on you for support will be covered as a dependent. Your children include: your biological children, your adopted children, your stepchildren and any other child you support who lives with you in a parent-child relationship.
- If both parents are employed by Jackson County Schools, only one parent may elect dependent child life coverage.
AGE REDUCTION RULE -
Your Life Insurance amount in force on the day before the first day of the month in which you reach age 70 will be reduced by: 35% at age 70; 55% at age 75; 70% at age 80. The reduction will take effect on the first day of the calendar month following the month in which you reach the age specified. If you become insured during or after the month in which you reach the above ages, your amount of Life Insurance will be the applicable percentage of the amount shown for your classification.
ACCELERATED DEATH BENEFIT -
If, while covered under this Plan for Life Insurance you become terminally ill or your spouse becomes terminally ill, you may request that Aetna pay an Accelerated Death Benefit.

PERMANENT AND TOTAL DISABILITY FEATURE (PREMIUM WAIVER) -
If you become permanently and totally disabled, your insurance may be extended. You will not have to make any further contribution. No premium payments will be required from your Employer.
You are permanently and totally disabled only if disease or injury stops you from working at:
- your own job; or
- any other job for pay or profit;
You must meet all of the following to be eligible:
- Your Life Insurance must be in force when you become permanently and totally disabled.
- You must be under age 60 when you are first permanently and totally disabled.
- Your permanent and total disability must have lasted for at least 9 months.
- You must furnish all proof when requested. Aetna has the right to examine you, at its expense, before approving the proof.
LIFE INSURANCE PORTABILITY -
Portability allows employees, spouses and/or children to continue their Term Life coverage on a direct bill basis if the employee terminates employment (excluding retirement) with the plan sponsor. By paying a portable term pool rate, employees can keep their group coverage amount, subject to certain limits, until they reach the age limit specified in the plan. Portability is optional and only available on Supplemental Life.
The terms of this provision apply only to an amount of Life Insurance which is fully contributory (i.e. you pay the total cost) and which:
- for you, is at least $5,000;
- for your spouse, is at least $1,000; and
- for your dependent child, is at least $1,000.
The Eligibility Age is:
- for you, 74;
- for your dependent spouse, 64; and
- for your dependent child, the age which is one year younger than the age at which he or she will cease to meet the Plan's definition of dependent.
CONVERSION OF LIFE INSURANCE -
If any of your Life Insurance ceases because your employment ceases or you are no longer in a class eligible for such insurance, or because of age, pension or retirement, the amount of insurance which ceases (or a lesser amount if desired) may be converted to an individual life insurance policy.
If any of your Dependent's Life Insurance ceases because your employment terminates or you are no longer in a class eligible for such insurance, or the dependent ceases to be a dependent, or because of a reduction in the dependent's life insurance for any reason, or you die, the amount of insurance which ceases for each dependent (or lesser amounts, if desired) may be converted to individual life insurance policies.

SCHEDULE OF BENEFITS
OPTIONAL EMPLOYEE LIFE INSURANCE -
Your choice of the following amounts: $10,000 increments up to a maximum of $150,000. You must complete an Evidence of Insurability form and receive approval from Aetna for any Employee Optional Life requests that exceed the guaranteed issue limits of $150,000. The guaranteed issue coverage is only available to new hires electing coverage within the first 31 days of hire or to employees electing coverage during the initial Aetna enrollment period of April 14, 2008 to May 31, 2008.
OPTIONAL DEPENDENT LIFE INSURANCE
$5,000 increments up to a maximum of $50,000 on your spouse;
Option of $5,000 or $10,000 on each of your eligible children
Optional Spouse and/or Child Life Insurance is available only to those eligible Employees who are insured for Optional Employee Life Insurance and is limited to 50% of the Employee’s approved optional life insurance coverage. You must complete an Evidence of Insurability form and receive approval from Aetna for any Spouse Optional Life amounts that exceed the guaranteed issue limit of $20,000. The guaranteed issue coverage is only available to the Spouse of a newly hired employee who elects coverage within the first 31 days of hire or to Employees electing Spouse coverage during the initial Aetna enrollment period of April 14, 2008 to May 31, 2008.
Employee Coverage Amounts Available:
$10,000 up to $150,000 (in $10,000 increments).
10 Month Employee Cost:
$2.40 per $10,000 of coverage, per month.
12 Month Employee Cost:
$2.00 per $10,000 of coverage, per month.
Spouse Coverage Amounts: $5,000 up to $50,000 (in $5,000 increments)
10 Month Cost:
$2.04 per $5,000 of coverage, per month.
12 Month Cost:
$1.70 per $5,000 of coverage, per month.
Child(ren) Coverage - $5,000 or $10,000
10 Month Cost:
$0.60 per $5,000 of coverage, per month (regardless of number of children).
12 Month Cost
$0.50 per $5,000 of coverage, per month (regardless of number of eligible children).
Additional Information Concerning your Life Insurance Benefits
Employees must have at least $10,000 optional coverage on themselves in order to purchase spouse or child (ren) coverage. Spouse / Child (ren) coverage is limited to 50% of Employee’s Optional Life coverage amount.
New employees have the opportunity to purchase any amount up to the guaranteed issue amount of $150,000 (for employee); $20,000 (for spouse); $5,000 or $10,000 (for children) without providing evidence of insurability, as long as enrollment occurs within the first 31 days of hire.
